That's the one, RS probably sells a similar looking cable in both the 50 and 75 ohm ratings.

That's one way that should work, assuming that by slave you mean it gets its clock from lightpipe.

The BNC sync cable should be internally terminated on the card, but I don't know more than that.

My guess is either improper cable, clock configuration, termination, or just a dodgy lightpipe implementation on the card. I hope you get it working.
