#2 is correct.The THRU acts as just that.Any info coming in the IN port can be affected and sent to the next device via the THRU. Info recieved at the IN port will affect your device and then pass on to the next one via the THRU.The OUT sends info to another device only.

An example would be:

Midi controller keyboard OUT to Sequencer IN. Sequencer OUT to Midi mixer THRU,Midi mixer THRU to Synth 1 IN,Synth 1 THRU to Synth 2 IN,Synth 2 THRU to Synth 3 IN.

This allows you to play and record midi data from multiple synths at the same time with out repatching.(the sequencer needs to have the midi echo turned on to here what you are playing)
