I had an identical problem, and this was the problem and solution:
On the circuit card inside, near the outputs, is a row of four transistors (Q7, 8, 9, and 10, I think). They are mutes that protect your amp/speakers when you turn on the unit. One of them shorted out. If you know how to measure transistors front to back with a multimeter, you can find out which one. To fix the fault, remove the transistor by clipping the leads. No need to replace it, just remember to keep your amp or mixer volume all the way down when you turn on your QS. Good luck!
