There are a number of reasons why this problem can occur (since I had this prob, I found out the hard way. I had a sb pci128 sound card, joystick to midi cable and casio keyboard that is general midi all the way.
The first thing to make sure is that your computer configuration is correct. The computer needs to be told to use the external port for both midi in and midi out. Since you didn't specify your operating system, I can't give you any specific configuration information.
After that, make sure you check your connections. If you have a spare cord that you know works, try it. Whatever you do, don't give up.
I found out my problem was the PCI128 sound card itself. I got a SB Live! card and the problem went away.
